If you derive any of your income from commissions or self-employment or own any rental real estate,
please submit copies of your 2002 and 2003 signed Federal income tax returns with all schedules and
W-2's. |

If you have 25% or greater ownership interest in a corporation, please submit copies of the 2002 and 2003
signed Corporate tax returns (Federal) with all schedules, W-2's and the documentation listed it item #2
above. |

If you have 25% or greater ownership interest in any partnerships, "S" corporations or trusts,
please submit copies of the 2002 and 2003 Federal income tax returns for each entity with "K-1" schedules
and the requested documentation from item #2 above. |

If you are obligated to pay either alimony or child support, please submit a copy of the complete, final
signed divorce decree with all stipulations and attachments. |

If you receive alimony and child support AND WISH IT TO BE CONSIDERED AS A BASIS OF REPAYMENT ON YOUR
MORTGAGE, please submit the documentation in item #5 with evidence showing receipt of the alimony and/or
child support for the past 12 months (for example - 12 months of cancelled checks). |

If you own any residential rental real estate (1-4 family), please submit copies of current signed leases
(as available) or signed and notarized tenant rental statements along with copies of the most recent real
estate tax bill(s) and mortgage statement(s) FOR EACH RESIDENTIAL RENTAL PROPERTY AND YOUR PRIMARY
RESIDENCE(S). |

If you are selling another property in order to purchase this home, please provide us with a copy of the
(real estate broker's listing agreement and, (if available) the fully signed Purchase and Sales Agreement
with all addenda. |
